If you have decided to become our customer, the first step is to register. To complete the registration, please provide the required information and documents by filling out the registration form.

What will we need?

  • Excerpt from the Commercial Register or Trade Licens
  • Tax ID (DIČ) or VAT ID (IČ DPH), if applicable
  • Bank Account Number
  • Delivery Address, if different from the company’s registered office
  • Contact Information (contact person, phone number, email)
  • Business/Working Hours during which you can accept delivery
  • Person Authorized to Accept Deliveries
  • Veterinary License (if you are a veterinarian)
  • Permit for Opiates / Controlled Substances, if you hold one
  • Permit for Scheduled Substances (Precursors), if you hold one
